Ideally patients will have read the program materials and filled out the IPPS questionnaire prior to their scheduled appointment.

 

                         There are two ways to refer your patients to DWC:

1. Call DWC's referral line at (480) 559 - 4776, Option 1 for Appointments
2. Print Patient Referral Form and fax to (866) 526 - 7086

Please fax pertinent patient care notes, History & Physical and insurance information to the referral fax line.
