AT DESERT WOMEN’S CARE

DWC offers a variety of services to patients depending on your precise needs and requests.   Be certain to note the patient's diagnosis on the referral form as well as the reason for the consultation. DWC physicians will communicate with you immediately following the consultation.

                         There are two ways to refer your patient to DWC:

1. Call DWC's referral line at (480) 559 - 4776, Option 1
2. Print Patient Referral Form and fax to (866) 526 - 7086

Please fax pertinent patient care notes, History & Physical and insurance information to the referral fax line
