Ideally the patient will be given handouts appropriate to her condition and will have read them prior to her appointment. Additionally, the opportunity to examine FOUR DAYS of a voiding diary at the initial visit will provide important insight into the patient's diagnosis(es).

 

                         There are two ways to refer your patient to DWC:

1. Call DWC's referral line at (480) 559 - 4776, Option 1
2. Print Patient Referral Form and fax to (866) 526 - 7086

Please fax pertinent patient care notes, History & Physical and insurance information to the referral fax line.
